Extension without change of a currently approved collection
Approved as amended by DOL's memoranda to OMB of 3/15/95 and 3/21/95. As DOL has agreed, approval through 9/97 will place this package on the same time frame as the related BLS form 1220- 0045. In addition, DOL expects to publish a proposed rule that would likely necessitate changes to this collection under 3504(h) of the PRA, and final revisions following publication of a final rule. DOL shall submit any requested changes in this form that result from the rulemaking process in a timely fashion.
