Abstract: This information collection is for identifying the need to replace a security badge for access to BPA facilities and critical infrastructure by BPA employees and contractors; requests for visitors to visit BPA facilities and contractors; and reports of incidents of loss or damage to BPA facilities. The forms gather the type of badge needed, employee or contractor contact information, and a brief description of the incident resulting gin the loss of the badge or damage to the badge or BPA property.
