Abstract: Revised forms have been made necessary by the conversion to an Internet-based data collection system -- the SCSEP Performance and Results QPR (SPARQ) system. In addition, regular program data collection forms needed to be modified to collect data for the Section 502(e) training providers. Finally, grantees have requested enhanced case management functionality.
