The National Register of Historic Places Registration Forms (NPS Form 10-900), the National Register of Historic Plates Registration Continuation Sheet (NPS Form 10-900-a), and the Multiple Property Submission/Multiple Property Documentation Form (NPS Form 10-900-b), are completed by individuals, SHPOs, FPOs, THPOs, or other nominating organizations for nomination to the National Register.
