The forms in this collection are used to obtain pertinent information to provide financial assistance, and if necessary, direct assistance to eligible individuals and households who, as a direct result of a disaster or emergency, have uninsured or under-insured, necessary or serious expenses they are unable to meet. To provide meaningful access to individuals with disabilities throughout FEMA programs, the revision to the collection will obtain pertinent disability-related information.
It is vital that the revisions to the currently approved information collection be implemented as soon as possible due to the Consolidated Appropriations Act, 2021 signed into law on December 27, 2020, which allocated up to two (2) billion dollars to provide financial assistance to individuals or households with COVID-19 related funeral expenses.

The burden hour change is a result of the Consolidated Appropriations Act, 2021 and the need for FEMA to submit an Emergency Request to implement a COVID-19 Funeral Assistance registration process. Specifically, the modified use of 009-0-1T, Tele-Registration as new instrument 009-0-1T-COVID-FA increased burden hours by 183,350 due to the estimated number of individuals who will apply for COVID-19 Funeral Assistance, and the RFI hours increased by 92,019 due to the documentation required to verify COVID-19 funeral expenses and to prevent a duplication of benefits.
