The pilot/operator accident and incident report form is an information collection tool. The form is completed by the pilot, the operator, or their representative who was involved in an accident or incident. The pilot/operator acquires and submits the form by email, mail, or fax. The form is available at www.ntsb.gov.
Under OMB Control Number 3147-0001, the NTSB uses Form 6120.1 to collect information about aviation accidents and incidents that aircraft operators must report to the NTSB. 49 U.S.C. 1132(b) requires the NTSB to promulgate regulations governing notification and reporting of aircraft accidents. 49 C.F.R. 830.5 requires an aircraft operator to immediately notify the NTSB about an aircraft accident, an aircraft incident listed in section 830.5(a), or an overdue aircraft that is believed to have been involved in an accident. 49 C.F.R. 830.15 requires an operator to file with the NTSB a report about the accident on Form 6120.1 within ten days after an accident, or within seven days after an overdue aircraft is believed to have been involved in an accident. The NTSB needs this information collection tool to launch to accidents that it investigates. The NTSB recently implemented a web-based form for aircraft operators to immediately notify the NTSB about an aircraft incident listed in section 830.5(a). The NTSB is evaluating the most effective method of collecting aviation accident and incident notifications and reports. The NTSB is also determining whether and how to consolidate accident and incident notification and reporting under a single OMB Control Number. Until the NTSB completes this evaluation, it must be able to receive notifications and reports in order to investigate aviation accidents and incidents.
