Short Statement: CBP proposes to revise Form 5106 by adding new data elements. The new data elements are described in paragraph one of this Statement and a draft of the new form will be included in this ICR. CBP proposes to increase the time per response from 15 minutes to 45 minutes as a result of adding these new data elements which increases the burden hours from 75,000 to 225,000.
