The Governor or a designated State agency (or State entity) is required to collect this information in order to determine eligibility of training providers and to maintain and to publicly disseminate the State Eligible Training Provider List. On August 31, 2015, OMB filed a comment in accordance with 5 CRF 1320.11(c) and assigned the control number of 1205-0523. This ICR requests approval of the information collection.
