Recruitment and enrollment took longer than originally anticipated – extending from 12 to 18 months in order to achieve the study's enrollment targets. As a result, the field period for each the follow-up surveys increased from 16 to 22 months in order to achieve sufficiently high response rates. The lengthier field period, coupled with the slow buildup of sample, increased overall data collection costs. Additionally, the aforementioned revisions to the 48-month survey, including the addition of five new questions and the removal of 16 questions designed to facilitate future contact (since this is the last survey administration and no further contact will be necessary) resulted in a reduced burden estimate than previously proposed. More specifically, our pretests for the 48-month survey found that the survey will take approximately 35 minutes (rather than the 40 minutes originally proposed).
