The purpose of this revision is to request approval to change one of the current forms and to extend collection for an additional three years. The forms are used to collect surveillance data on antimicrobial resistance in Neisseria gonorrohoeae. These data are used by public health officials at the national, state, and local level to determine appropriate therapy for gonorrhea. In addition, the data are used to monitor trends N. gonorrhoeae antimicrobial resistance and certain epidemiologic aspects of gonorrhea.
