CDC provides funding to chronic disease prevention and control programs (DPCPs) in 50 states, the District of Columbia, Puerto Rico, and the Virgin Islands. CDC collects program and progress information from programs to monitor performance and support program needs for technical assistance. CDC is transitioning to an enhanced MIS a common set of performance indicators.

This is a new information collection involving an enhanced Management Information system. The new MIS will harmonize reporting requirements across multiple state-based programs and replace two previously approved Management Information Systems, 0920-0601, and 0920-0479.
