CDC provides funding to chronic disease prevention and control programs (DPCPs) in 50 states, the District of Columbia, Puerto Rico, and the Virgin Islands. CDC collects program and progress information from programs to monitor performance and support program needs for technical assistance. CDC is transitioning to an enhanced MIS a common set of performance indicators.
