This request for revision of electronic collection of information (OMB 0920-0650, exp. 8/31/2010) is from the Prevention Research Centers (PRC) Program of CDC. This revision includes change to data collection methods, modify title, reduce the number of questions and propose collecting data only annually, and increase the number of respondent PRCs. PRCs are housed in schools of public health or medicine. They conduct health promotion and disease prevention research on a range of topics using a multi-disciplinary and community-based approach. Partners collaborate with the PRC to assess community health priorities, identify research priorities, set a research agenda, conduct research projects and related activities such as training and technical assistance, and disseminate research to public health practitioners, researchers, and the general public. The PRC Information System (IS) is a web-based application to collect information related to work plans and progress reports and to track progress toward the PRC performance indicators.

This request is for a revision, with an overall decrease in burden and increase in respondents. The program requests approval by June 1 so they can provide some training on new procedures before the first data collection cycle. The program would like to change the title to "Prevention Research Centers Program National Evaluation Reporting System".
