This information collection request is used by the FMCSA to help ensure that drivers, motor carriers, and the States are complying with the notification and recordkeeping requirements for information related to testing, licensing, violations, convictions, and disqualifications. It is being revised due to additional requirements in a Final Rule entitled, "Medical Certification as Part of the CDL," under RIN 2126-AA10.
