CDC requests permission to not print the OMB expiration date on the 2008-2009 National Hospital Discharge Survey (NHDS) abstract form. The NHDS provides detailed information on characteristics, diagnoses, and surgical and other procedures for the patients discharged from short-stay non-Federal hospitals in the United States. The information collected is available in written reports, in unpubished form through special tabulations, on public use files, CD-ROMS and on-line at the NCHS website.
