NRC regulation require NRC licensees to report incidents and events involving the use of radioactive by-product material, and saource material, such as thos involving a radiation over- exposures, leaking or contaminated sealed source(s), release of excessive contimination of radioactive material, lost or stolen radioactive material, equipment failures, and abandoned well logging sources. NRC is requesting that the Agreement States provide information on the initial notification, response actions, and follow-up investions on all events involving the use of nuclear materials regulated pursuant to the AEA.

Reason for change in burden is due to agency re-estimation of the number of burden hours previously submitted to OMB from 1252 to 1240 hours. Also, the total number of responses decreased from 626 to 620. It appears as though a typographical error was made when this package was submitted to OMB in 2003. This change will correct this error.
