This Information Collection is cleared conditionally upon the associated DoD form, DD Form 370, being revised to accurately reflect the routine uses contained in the companion systems of records. The revised description of the routine uses shall be sufficient to adequately understand what the information being provided may be used for while allowing for the limitations of DD Form 370's size. DoD has 30 business days from the date of clearance of this information collection to provide OIRA with the revised DD Form 370.
