Abstract: The purpose of this data collection is to assess the prevalence of self-reported risk behaviors associated with cardiovascular disease, deficits in breast and cervical cancer screening, and diabetes. Data will be collected on a yearly basis enabling investigators to assess changes in the performance measures during the intervention and community progress toward meeting performance objectives.
