Abstract: The Form Ad-2016, USDA Registration From to Request Electronic Access Code is a registration form, used by the CPSs to provide single sign on to web form applications for USDA customers and employees. The form AD-2016 enables USDA customers to request an account at their local USDA Service Center, by mail, or by fax. After the USDA customer's identiy is confirm, the customer obtains a User ID and Password, whch they use to conduct business on many USDA web sites.
