Terms of Clearance: Approved for one year. OMB asks the DOJ Clearance Office to determine if the use of this form, earlier put in place without clearance,is causing any significant problems. We would also like evaluation of the burden of certain elements such as provision of fingerprinting and photographs for larger number of respondants. Costs in question 13 have been doublecounted and some are inappropriately included (fees). Another round of notices will not be necessary to extend the one year clearance an additional two years.
