Approved consistent with final draft of form, faxed on 8/9/01. This form uses the long version of the OPM disability codes, and asks all individuals to write in the code that best describes them. This form was in violation of the PRA. INS shall insure that no such further violations occur in the future. The form is cleared until March 2003, at which point the OPM race and ethnicity codes should be updated to reflect OMB guidelines. Upon resubmission, INS shall update the race/ethn. codes accordingly.
