Terms of Clearance: Approved for 9 months, during which time FNS will do the following in developing new forms for this program: (1) FNS will submit a seperate package to conduct focus group testing of the new forms. This focus group testing should be completed before the new forms are submitted to OMB for review. (2) FNS will submit a revised 252 form for clearance, but will not implement the use of the new form until the STARS-II system is operational. At the time of submission of the new form, FNS will re-clear the current form for use until the new form is ready for use. (3) FNS will continue to explore revisions to the 252-2 form, and will update OMB on progress or plans for revisions upon submission of the revised package. Original burden calculations submitted with this package did not account for the store visits for reauthorizations. As per memo from FNS dated 8/12, there are an additional 19,891 stores that go through the store visit reauth. process, with a burden estimate of 1 minute of public burden per store, for an overall burden of 397 hours. This burden has been added to the estimates submitted for the new annual application burden previously submitted.
