The Statement of Consent will be used in conjunction with a DS-11 only in cases of passport issuance to a minor under the age of 14. It will be completed either by the non-appearing parent to give his or her statutorily required consent to the issuance of a passport to the minor; or, by the appearing parent to explain the absence of the other parent's consent.
