Abstract: OPM Form 805 is used to elicit information from applicants wishing to register to vote. Using the information, the OPM examiner will determine voting eligibility and, if the applicant is found to be eligible, the applicant's name will be certified to state officials for addition to the official voting lists. The voter eligibility is determined by state law requirements. Without this information eligible voters many be denied their opportunity to vote.
