Abstract: This is a request for OMB approval of a new package consisting of two updated forms which before now had been part of the tuberculosis (TB) surveillance package approved under OMB No. 0920-0026. The Division of Tuberculosis Elimination (DTBE), National Center for STD, HIV, and TB Prevention is seeking a new and separate OMB approval to reflect the changes in the content and the administration of the reports.
