Terms of Clearance: This Information Collection Request (ICR) is again being approved for only 180 days. The agency is instructed once again to verify the hourly-burden-per-respondent for this ICR, including a more accurate breakdown for each form and a clearer description of which forms are required for which kinds of applications. In addition, the agency must, prior to next submission, revise the forms to reflect new standards for race and ethnicity reporting, add a Privacy Act Statement for those instances where the Privacy Act of 1974 applies, a description of how proprietary information is designated and protected, and modify the Paperwork Reduction Act statements on all forms to reflect the additional requirements of the Paperwork Reduction Act of 1995.
