Terms of Clearance: Approved as amended by DOL's memo to OMB of 12/16/97. In addition, OMB makes the following requests - that each year, DOL send to OMB a copy of the annual report (generated by data gathered by this form) sent to Congress and referred to in the 12/16 memo. OMB further requests that each quarter, DOL send to OMB a copy of the quaterly reports (generated by data gathered by this form) developed by DOL and used to monitor program activities and referred to in the 12/16 memo. It should be noted that OMB's reasons for requesting these reports are two-fold - first, to better assess the quality of the form itself and the data it generates; second, to better understand the underlying policy issues related to the relevant employment and training programs.
