This submission contains four versions of an instrument to be used in data collection for the summative evaluation of the Eisenhower National Clearinghouse (ENC) dissemination model. Subjects for two of the surveys will be selected through stratified random sampling of U.S. schools to obtain representative samples of principals and teachers, the largest target audience for ENC information and resources. The other two surveys will target known users of ENC services, the individuals being subclassified as single- and multiple-instance users. The instruments will be distributed by mail.
