Abstract: THE PURPOSE OF THE LTC IS TO OBTAIN INFORMATION ABOUT HEALTH CONDITION THAT AFFECT OLDER AMERICANS' EVERYDAY ACTIVITIES, ANY SPECIAL HEALTH CARE NEEDS OR SERVICES REQUIRED, AND ON THE PERSONS AND ORGANIZATIONS THAT MAY PROVIDE CARE. PLANNERS WILL USE THE INFORMATION TO DETERMINE THE HEALTH CARE NEEDS OF PEOPLE 65 YEARS OLD AND OVER.
