Terms of Clearance: This questionnaire is approved for use through August 31, 1994, to enable SBA to examine whether using a single form and sampling method- ology would improve the evaluation of its counseling and training programs. (Currently, the agency uses four such forms approved under four separate OMB control numbers.) Following this assessment, SBA should prepare a single clearance request covering the remaining form or forms or seek a generic clearance under the recent OMB guidanc for customer surveys.
