Abstract: THIS FORM IS REQUIRED BY REV. PROC. 66-11 AS AN ATTACHMENT TO THE TAX RETURN. THE FORM PROVIDES A STANDARD METHOD OF COMPUTING AND REPORTIN WATER DEPLETION DEDUCTIONS BY TAXPAYERS WHO EXTRACT GROUND WATER FROM THE OGALLALA GEOLOGICAL FORMATION. THE INTERNAL REVENUE SERVICE USES THE INFORMATION TO DETERMINE IF THE DEPLETION HAS BEEN COMPUTED CORRECTLY.
