Terms of Clearance: Approved on the condition, to which DOL has agreed, that the health questions which appear on both this form and the Job Corps Data sheet, OMB #1205-0025, will be deleted from the data sheet with the next printing of the Data Sheet one year from now. DOL will immediately se a policy memorandum to all Job Corps offices using the Data Sheet, whi instructs those offices to skip questions 24 and 25 which are collecte on this Health Questionnaire. DOL shall send a copy of this memorandu to OMB for the official file. In addition, DOL shall submit an inventory correction worksheet to revise the expiration date on the Da Sheet from 6/96 to 9/94, consistent with DOL's intent to resubmit the Data Sheet in one year.

Abstract: THE HEALTH QUESTIONNAIRE IS USED TO OBTAIN THE HEALTH HISTORY OF APPLICANTS TO THE PROGRAM TO DETERMINE MEDICAL ELIGIBILITY. THE APPLICANT MUST NOT HAVE A HEALTH CONDITION WHICH REPRESENTS A POTENTIALLY SERIOUS HAZARD TO THE YOUTH OR OTHERS, RESULTS IN A SIGNIFICANT INTERFERENCE IN THE NORMAL PERFORMANCE OF DUTIES, OR REQUIRES FREQUENT EXPENSIVE OR PROLONGED TREATMENT. THE ETA 6-82 IS
