Abstract: THE AUTHORIZATION AGREEMENT FOR ELECTRONIC FUNDS TRANSFER IS A FORM COMPLETED BY THOSE MEDICARE PROVIDERS, PHYSICIANS, AND SUPPLIERS THAT QUALIFY FOR MEDICARE PAYMENTS VIA ELECTRONIC FUNDS TRANSFER. MEDICARE CONTRACTORS WILL USE THE INFORMATION AS A RECORD OF AN OFFICIAL REQUES TO BEGIN PAYING THESE PROVIDERS, PHYSICIANS, AND SUPPLIERS VIA ELECTRONIC FUNDS TRANSFER.
