WHEN SAFETY DEFECTS ON SELF-PROPELLED EQUIPMENT ARE NOT CORRECTED IMMEDIATELY, THE DEFECT MUST BE REPORTED TO THE MINE OPERATOR WHO IS REQUIRED TO MAKE A RECORD OF THE REPORTED DEFECTS.
