Abstract: THE USE OF THIS FORM WAS DISCUSSED WITH IRS OFFICIALS BY THE SUGGESTER OFFICE AND WAS DESIGNED ACCORDINGLY. THIS INFORMATION IS NEEDED, ESPECIALLY WHEN THERE IS A CONFLICT BETWEEN THE CREDIT REPORT AND THE APPLICANT'S ASSERTION ABOUT THE STATUS OF TAXES.
