Abstract: THE INFORMATION OBTAINED IN THIS SURVEY OF COMMUNITY AGENCIES WHICH HA REQUESTED THE HEALTH RISK APPRAISAL COMPUTER PROGRAM DEVELOPED BY THE CENTERS FOR DISEASE CONTROL AND OF OTHER COMMUNITY AGENCIES WHICH PROVIDE RISK REDUCTION PROGRAMS FOR ADULTS WILL BE USED AS PART OF A PROGRAM EVALUATION STUDY SPONSORED BY THE CENTER FOR HEALTH PROMOTION AND EDUCATION, CDC.
