Abstract: THIS FORM PROVIDES DATA USED BY REA TO MEASURE THE DEGREE OF ITS ELECTRIC AND TELEPHONE BORROWERS' VOLUNTARY INITIATIVES IN RURAL COMMUNITY DEVELOPMENT AND SERVICES. IT IS A DEVICE FOR MONITORING, REPORTING, AND EVALUATING AGENCY PROGRAMS AND THE MANNER IN WHICH BORROWERS FULFILL THEIR USDA RURAL DEVELOPMENT RESPONSIBILITIES.
