Abstract: SCORE PROVIDES MANAGEMENT COUNSELING TO SMALL BUSINESSES. THE 12,000 MEMBERS ARE ORGANIZED INTO 400 CHAPTERS. THERE IS A 20 PERCENT TURN OVER IN MEMBERSHIP ANNUALLY. SBA DISTRICT OFFICES USE THE FORM TO KEE AN ACCURATE LIST OF ALL SCORE CHAPTERS AND THEIR ACTIVITIES.
