Abstract: THIS FORM SERVES AS A SUPPLEMENT TO THE DAMAGE SURVEY REPORT WHERE IT IS NECESSARY TO PROVIDE MORE DETAILED INFORMATION SPECIFICALLY ON BRID DAMAGE SUSTAINED DURING A DISASTER. THE DAMAGE SURVEY REPORT WHILE GENERALLY PROVIDING SUFFICIENT DETAIL ON MOST DAMAGES WAS FOUND TO BE LACKING IN SUPPORT OF CLAIMS FOR RESTORATION OR REPLACEMENT OF BRIDGES IT WOULD NOT BE PRACTICAL TO COMBINE THE DATA INTO A SINGLE FORM.
