SHEET 2 OF THIS FORM RECORDS THE BEER PRODUCED, RECEIVED, BOTTLING AND RACKING OPERATIONS. IT ALSO SHOWS BEER REMOVED FROM THE BREWERY AND IN WHAT MANNER (TAXABLE, NONTAXABLE). SHEET 1 IS USED AS BASIS FOR THE MONTHLY STATISTICAL REPORT PREPARED BY THE BREWER.
