IT IS USED TO NOTIFY THE REGISTRANT OF REFERRAL TO TRAINING AND ENTITLEMENT TO PAYMENTS, THE TRAINING FACILITY OF REGISTRANT'S REFERRAL AND THE PAYMENT UNIT OF SUCH REFERRAL.
