Abstract: THIS FORM PROVIDES PROGRAM PERSONNEL IN THE ACADEMIC ASSISTANCE DIVISION OF THE OFFICE OF CRIMINAL JUSTICE EDUCATION AND TRAINING, WITH THE INFORMATION THAT THEY NEED TO DESK MONITOR PROGRAM ACTIVITY, TO ESTIMATE THE AMOUNT OF FUNDS REQUIRED TO SATISFY THE NEEDS OF RETURNING STUDENTS AND MOST IMPORTANTLY, PROVIDE THEM WITH WITH PROGRAM ACTIVITY DATA THEY WILL REQUIRE IN MAKING JUDGEMENTS RELATIVE TO APPLICATIONS FOR FUTURE FUNDING.
