Abstract: APPROPRIATE FLOOD INSURANCE IS REQUIRED UNDER PL 93-234 FOR PUBLIC BUILDINGS AND MOBILE HOMES AS A PREREQUISITE TO PROJECT APPLICATION APPROVAL FOR PERM. WORK. THE DATA COLLECTED ON THIS FORM WILL ASSIST THE REGIONAL DIRECTOR IN FOLLOW-UPS TO INVESTIGATE THE REGUIREMENTS FOR ADDITIONAL INSURANCE AND TO OBTAIN PROOF OF INSURANCE, OR ASSURANCES, PRIOR TO APPROVAL OF A PROJECT APPLICATION FOR PERMANENT RESTORATIVE WORK
