Abstract: THE BASIC REQUIREMENT FOR THIS FORM STEMS FROM THE DISASTER RELIEF ACT OF 74, PL 93-288, AND FEDERAL DISASTER ASSISTANCE REGULATIONS, TITLE 44, PART 205.
