IT ALSO IS USED AS A REPORTING FORM, AS THE IMPORTED PRODUCT IS LISTED ON THE FORM AS TO QUANTITY, TYPE OF PRODUCT, COUNTRY OF ORIGIN AND ITS DISPOSITION - WHETHER PERMITTED OR REFUSED OR REFUSED ENTRY.
