Abstract: THE PURPOSE OF THIS FORM IS FOR THE COMPANY TO LET US KNOW THE LOCATION OF THE FREEZER PLANT TO AUTHORIZE FSQS INSPECTORS TO ENTER IT TO MAKE THE REQUIRED PRODUCT INSPECTION; AND TO MAKE CERTAIN THE PLANT AGREES TO HANDLE THE PRODUCT SO AS TO PREVENT SPOILAGE.
