Abstract: THIS FORM PROVIDES THE REA BORROWER, BORROWER'S ENGINEER CONTRACTOR AND REA FIELD ENGINEER WITH A RECORD OF THE RESULTS TEST PERFORMED ON TELEPHONE CONTROL OFFICE EQUIPMENT INSTALLATIONS BEING FINANCED BY REA. IT SPECIFICALLY INDICATES THOSE CORRECTIONS THAT MUST BE MADE BEFORE FINAL ACCEPTANCE OF EQUIPMENT AND FINAL ADVANCE OF REA LOAN FUNDS FOR FULL PAYMENT TO THE MANUFACTURER.
