INSTRUCTIONS FOR COMPLETING THE LOGBOOK FORM Start a new logbook page for every trip. Please fill out a line for each day of fishing effort; each page can be used for up to 4 days of fishing effort. Trip Section: vessel name, captain, captain commercial fishing license number, Coast Guard vessel ID or State Registration Number, observed trip (yes or no), departure port, departure date, arrival port, arrival date. Landing Section: port landed, landing receipt number(s), permit type (circle one) Fishing Effort Section: FOR EACH INDIVIDUAL DAY ENTER STANDARD (UNLINKED BUOYS) GEAR IN THE TOP WHITE LINE AND LINKED GEAR IN THE SECOND GRAY LINE.
