FARM = OPERATING PLAN=20 FOR AN INDIVIDUAL =96 2009 and Subsequent = Years

This form = is used to=20 collect information about individuals that is used by FSA to determine=20 eligibility for payments. = This form=20 is designed for individuals using a social security number and = requesting=20 program payments as an individual on their own farming=20 operation.

Submit = the original=20 of the completed form in hard copy or facsimile to the appropriate USDA=20 servicing office.

Customers = who have=20 established electronic access credentials with USDA may electronically = transmit=20 this form to the USDA servicing office, provided that (1) the customer=20 submitting the form is the person required to sign the transaction, or = (2) the=20 customer has an approved Power of Attorney (Form FSA-211) on file with = USDA to=20 sign for other customers for the program and type of transaction = represented by=20 this form.
